1.　この個人情報保護方針における「個人情報」とは、「個人情報の保護に関する法律」で定められている「個人情報」をいい、以下の情報が「個人情報」に含まれます。生存する個人に関する情報で、特定の個人を識別することができるもの（他の情報と照合することによって個人を識別できる場合を含みます。）個人識別符号（マイナンバー、パスポート番号、免許証番号、年金番号、保険者番号、顔認証データ、指紋認証データなど） 2.　当社が取得する個人情報には、例えば、以下のようなものがあります。氏名、性別、生年月日、住所、郵便番号、電話番号、ファックス番号、電子メールアドレス勤務先の名称、所属部署名、役職名、住所、郵便番号、電話番号、ファックス番号、電子メールアドレスクレジットカード情報、銀行口座情報パスポート番号、免許証番号、年金番号、保険者番号その他の身分証明書番号商品の購入履歴、サービスの利用履歴、ウェブサイトへの通信履歴（閲覧履歴、検索履歴等）病歴、健康診断や検査の結果、質問等への回答結果、診療や薬剤の処方に関する記録、その他の健康情報（以下「健康情報」といいます。）撮影・録音された個人の画像や音声会員資格に関する情報（会員の有無、入退会の履歴等） 3.　「個人情報」には、「統計情報」は含まれません。「統計情報」とは、複数の個人の情報から、商品・サービス・会員等のグループやカテゴリーごとに共通する要素を抽出して数値化したデータで、特定の個人に関する情報を含まないものです。「統計情報」は、当社がトレンドと会員等のニーズを理解するために用いられ、このことにより、当社は新しい商品やサービスについてより良い検討を行うことができるとともに、現存する商品及びサービスを会員等の要望にお応えするよう改善できることになります。 B.　個人情報の取得 1.　当社は、適法かつ適正な手段により、会員等の個人情報を取得します。当社が取得する個人情報には、会員等のご本人の個人情報に加えて、会員等を通じて間接的に取得したご家族等の個人情報も含まれます。 2.　当社は、主に以下の場合に個人情報を取得します。当社の会員に登録する場合、又は当社の会員資格を更新する場合当社の会員サービスに登録し、または参加される場合当社の商品やサービスを注文し、または購入する場合アンケート等に回答する場合当社の倉庫店等に来店する場合当社の会員サービス、イベントまたは販売促進キャンペーンに関するお問合わせや連絡をいただく場合薬局、倉庫店等で質問表等に記入する場合当社以外の第三者から委託、事業承継または共同利用により個人情報が提供される場合 3.　要配慮個人情報の取得（1）「要配慮個人情報」とは、ご本人の人種、信条、社会的身分、病歴、犯罪の経歴、犯罪により害を被った事実その他本人に対する不当な差別、偏見その他の不利益が生じないようにその取扱いに特に配慮を要する記述等が含まれる個人情報をいいます。（2）当社は、以下の場合を除いて、あらかじめご本人の同意を得ないで要配慮個人情報を取得しません。法令に基づく場合人の生命、身体又は財産の保護のために必要がある場合であって、ご本人の同意を得ることが困難であるとき。公衆衛生の向上又は児童の健全な育成の推進のために特に必要がある場合であって、ご本人の同意を得ることが困難であるとき。国の機関若しくは地方公共団体又はその委託を受けた者が法令の定める事務を遂行することに対して協力する必要がある場合であって、ご本人の同意を得ることにより当該事務の遂行に支障を及ぼすおそれがあるとき。要配慮個人情報が、ご本人、国の機関、地方公共団体等により公開されている場合ご本人を防犯カメラ等により撮影する場合要配慮個人情報を委託、事業の承継または共同利用により取得する場合（3）当社は、会員等から会員等のご家族の要配慮個人情報を取得する場合には、原則として、ご家族の同意を得て取得します。なお、会員等のご家族が年少、ご高齢、ご病気等のために同意するかどうかについて判断できる能力がない場合又は会員等のご家族が会員等に対して当社に要配慮個人情報を提供することへの承諾を与えている場合には、会員等から同意を得て要配慮個人情報を取得する場合があります。 C.　個人情報の利用目的 1.　当社は、会員等の個人情報（健康情報については、２のとおり）を、以下の目的の範囲内で利用します。会員等からの商品やサービスの注文への対応・確認・記録会員等が購入した商品の配送会員等向けのイベント、販売促進キャンペーン、割引等のプロモーションに関する情報（以下「プロモーション情報」といいます。）を提供するため第三者が提供する会員等向けの特別の商品やサービス（以下「会員サービス」といいます。）を紹介、提供するため会員等のご意見やご要望を把握するためのアンケート等の実施クレジットカード等による代金決済、年会費等の自動引き落としのため商品のリコールまたは安全上の問題について会員等に警告するため商品の不具合に関するお問い合わせ、修理、交換に対応するため会員等からのお問い合わせ等に対応するため商品やサービスの企画・開発・提案のための個人情報の分析会員等が個別に同意した目的に利用するためその他、当社がより良いサービスを提供することができるように会員等のニーズを理解するため当社が会員等と締結している契約に関する義務の履行や契約関係の維持・管理等のため当社、当社の会員、当社の従業員もしくは第三者の生命・身体の安全、財産その他の権利を保護するため 2.　当社は、会員等の個人情報のうち健康情報については、以下の目的の範囲内で利用します。調剤サービス及び薬剤情報のご提供健康維持及び増進等に有益と思われる情報のご提供保険請求事務投薬に際してのご家族等に対する情報提供を行うため調剤サービス及び保険請求事務処理に際し、処方箋発行元の医療機関又は医師へ照会するため病院、診療所、薬局、訪問看護ステーション、介護サービス事業者などとの必要な連携を図るため保険請求事務に係る審査支払機関に対する調剤報酬明細書の提出のため保険請求事務に係る審査支払機関又は保険者からの照会に対する回答のためメガネ、コンタクトレンズ、補聴器等の作成、提供、メンテナンス等のため会員等からのお問い合わせ等に対応するため情報分析等の研究のため（個人を特定できないように匿名加工した情報として利用します）当社が会員等と締結している契約に関する義務の履行や契約関係の維持・管理等のため当社、当社の会員、当社の従業員もしくは第三者の生命・身体の安全、財産その他の権利を保護するためＤ.　個人情報の第三者提供 1.　本人の同意に基づく第三者提供当社は、以下の場合、ご本人の同意に基づいて、「Ｃ．個人情報の利用目的」の範囲内で、会員等の個人情報を第三者に提供することがあります。（委託または共同利用に伴って個人情報を委託先や共同利用者に提供する場合については、「Ｅ．個人情報の取扱いの委託」及び「Ｆ．個人情報の共同利用」をご覧ください。）会員等が購入した商品のベンダー（販売会社）・サプライヤー（製造会社）等に、送付先や請求先の住所、電子メールアドレス、電話番号及び商品オーダー等の、会員等が注文する商品又はサービスを提供するために必要な個人情報を提供する場合会員等からのお問い合わせ等に対応するため、ベンダー・サプライヤー等に連絡・照会等をする場合ご本人に代わって個人情報を第三者に提供する場合（例）・会員等がクレジットカードの発行を申し込むために、当社を通じてクレジットカード会社に申込みに必要な個人情報を提供する場合・クレジットカードで代金を決済するのに伴い、当社を通じてカード番号や売上・取引に関する情報等をクレジットカード会社や個人信用情報機関に提供する場合・年会費等の自動引き落としのためにシステム会社に必要な個人情報を提供する場合「Ｃ．個人情報の利用目的」・２に記載した利用目的のために、健康情報を他の病院、診療所、薬局、訪問看護ステーション、介護サービス事業者、審査支払機関または保険者等に提供する場合その他、「Ｃ．個人情報の利用目的」記載の利用目的を達成するために第三者に個人情報を提供する必要がある場合外国にある第三者に、1)から5)の事由により個人情報を提供する場合外国にある第三者に、「Ｅ．個人情報の取扱いの委託」記載のとおり個人情報の取扱いを委託する場合外国にある第三者との間で、「Ｆ．個人情報の共同利用」記載のとおり個人情報を共同利用する場合会員等が個別に第三者への提供に同意した場合（※上記6)～8)の「外国」には、当社のウェブサイト（http://www.costco.co.jp/p/aboutcostco/worldwide）に掲載されている国内外の当社グループ各社の所在国・地域、シンガポールが含まれます。また、クラウドサービスの利用等に伴いこれら以外の外国にある第三者に個人情報を提供する場合があります。） 2.　法令に基づく場合などの第三者提供当社は、以下の場合に、ご本人の同意を得ないで、個人情報を第三者に提供することがあります。法令に基づく場合人の生命、身体又は財産の保護のために必要がある場合であって、本人の同意を得ることが困難であるとき（例えば、会員等が当社で購入した商品に製品事故が生じて、製造メーカーよりリコールの要請があった場合に、その商品を購入した会員等の連絡先等の情報を製造メーカーに提供する場合など。）公衆衛生の向上又は児童の健全な育成の推進のために特に必要がある場合であって、本人の同意を得ることが困難であるとき国の機関若しくは地方公共団体又はその委託を受けた者が法令の定める事務を遂行することに対して協力する必要がある場合であって、本人の同意を得ることにより当該事務の遂行に支障を及ぼすおそれがあるときＥ.　個人情報の取扱いの委託 1.　当社は、会員等の個人情報の取扱いを外部業者に委託することに伴い、個人情報を委託先に提供することがあります。その場合、適切な委託先を選定し、委託先に対して、委託された個人情報の安全管理が図られるように指示、監督します。 2.　当社が個人情報の取扱いを外部業者に委託する場合には、例えば、以下のような場合があります。個人情報のデータ入力・管理・処理の委託商品の配送の委託ダイレクトメールの送付の委託電子メールの送信の委託会員サービスの企画・開発・提案のための個人情報の分析の委託年会費等の決済事務の委託会員等からの商品やサービスの注文への対応・確認・記録Ｆ.　個人情報の共同利用当社は、「Ｃ．個人情報の利用目的」の範囲内で、以下のとおり、国内外の当社の関連会社との間で会員等の個人情報を共同利用することがあります。共同利用する個人情報の項目・「Ａ．個人情報の定義及び項目」記載の個人情報共同利用者（関連会社）の範囲当社のウェブサイト（http://www.costco.co.jp/p/aboutcostco/worldwide）に掲載されている国内外の当社グループ各社共同利用の目的「Ｃ．個人情報の利用目的」記載の利用目的個人データの管理責任者当社の個人情報担当責任者Ｇ.　事業の承継に伴う個人情報の提供当社は、合併、会社分割、事業の譲渡、営業資産の譲渡その他の事業の承継（以下「事業の承継」といいます。）に伴って当社が保有する個人情報を事業の承継の相手方に提供する場合があります。また、事業の承継に先立つ交渉過程において、適切な情報管理の措置を講じた上で個人情報を事業の承継の相手方に提供する場合があります。Ｈ.　安全管理措置当社は、個人情報が当社の管理下にある間の、喪失、誤使用又は改ざんから個人情報を保護するよう、安全管理措置に努めています。当社が収集する個人情報は、電磁的に保管されており、他の会員情報と組み合わされることがあります。当社は、無権限のアクセスから個人情報を保護するよう、技術上、契約上、管理上及び物理上の措置を取るよう努めています。Ｉ.　オンラインに特有の取扱い 1.　クッキーとは何か、クッキーを認めることが必要かクッキーは、会員等のコンピュータに保存される小さなファイルです。costco.co.jpにおいて、当社は、会員等の履歴を最適化するために、クッキーを使用しています。costco.co.jpは、いかなる個人情報もクッキーにより保存することはありません。 costco.co.jp をブラウズするクッキーの選択は自由です。 2.　他のウェブサイトへのオンライン上のリンク会員サービスおよび当社ウェブサイトの選択された他の部分を通じて、会員等は、当社の会員等に商品およびサービスを提供することに同意した第三者の他のウェブサイトにリンクすることができます。会員等が当該リンクされたページで提供された個人情報は、当該第三者に直接提供され、当該第三者のプライバシーポリシーが適用されます。上記に記載された場合を除き、当社は、当社がリンクするウェブサイトのコンテンツ、プライバシーおよびセキュリティの運用、ポリシーにつき責任を負いません。当社のサイトから第三者のサイトや他のサイトへのリンクは、会員等の利便性のために提供されます。会員等は、当該第三者に個人情報を提供する前に、当該第三者の個人情報保護及びセキュリティに関する取扱い及び方針に精通されることをお勧めします。 3.　当社がオンライン上で収集する情報の保有会員等のオンラインでの活動の結果として当社が自動的に収集する情報は、一般には個人を特定できる形で保有されることはなく、もし当該情報が個人を特定できるものである場合には、当社は当社のウェブサイトに関連する研究および分析を行うために必要な期間のみ、これを保有します。ただし、特定の取引やイベントに関連する不正や問題を調査するために必要な情報については、この限りではありません。 B.　Acquisition of personal information 1.　Our company acquires personal information of our members or the like by legitimate and appropriate means. Personal information that our company acquires includes not only personal information of our members or the like themselves, but also personal information of their families, etc. that our company indirectly acquires through our members or the like. 2.　Our company acquires personal information mainly in the following cases. When a person is registered as a member of our company, or when a person’s membership of our company is renewed When a person is registered for our company’s member services, or participates in the services When a person places an order for, or purchases, our company’s product or service When a person replies to a questionnaire or the like When a person visits our company’s warehouse or the like When a person makes an inquiry on our company’s member services, events or promotional campaigns When a person fills in a questionnaire or the like at a pharmacy, warehouse, etc. When a third party other than our company provides personal information on a commissioned basis, or through business succession or joint use 3.　Acquisition of special-care required personal information （1）“Special-care required personal information” refers to personal information comprising the principal’s race, creed, social status, medical history, criminal record, or the fact of having suffered damage by a crime, or other descriptions, etc. as those of which the handling requires special care so as not to cause unfair discrimination, prejudice or any other disadvantages to the principal. （2）Our company does not acquire special-care required personal information without obtaining the principal’s consent in advance, except in the following cases. Cases based on laws and regulations Cases in which there is a need to protect a human life, body or fortune, and when it is difficult to obtain the principal’s consent Cases in which there is a special need to enhance public hygiene or promote fostering healthy children, and when it is difficult to obtain the principal’s consent Cases in which there is a need to cooperate in regard to a central government organization or a local government, or a person entrusted by them performing affairs prescribed by laws and regulations, and when there is a possibility that obtaining a principal’s consent would interfere with the performance of the said affairs Cases in which the special-care required personal information is being open to the public by a principal, a central government organization, local government or the like Cases in which the principal is photographed with a security camera or the like Cases in which the special-care required personal information is acquired on a commissioned basis, or through business succession or joint use （3）If our company acquires special-care required personal information of a family of our member or the like from the member or the like, our company obtains the family’s consent in advance in principle. Personal information that our member or the like provides on such linked page is directly provided to such third party, and a privacy policy of the third party will be applied. Except for the cases indicated above, our company will not assume responsibility for the contents, privacy and security operation and policy of the websites that our company links. The links from our company’s website to third-party sites and other sites are provided for convenience of our members or the like. この個人情報保護方針は、コストコホールセールジャパン株式会社（以下「当社」といいます）の個人情報の取扱いに関する方針を説明するもので、当社が取得する全ての方（会員および非会員の双方。以下「会員等」といいます。）の個人情報に適用されます。

A.　個人情報の定義及び項目
1.　この個人情報保護方針における「個人情報」とは、「個人情報の保護に関する法律」で定められている「個人情報」をいい、以下の情報が「個人情報」に含まれます。
生存する個人に関する情報で、特定の個人を識別することができるもの（他の情報と照合することによって個人を識別できる場合を含みます。）
個人識別符号（マイナンバー、パスポート番号、免許証番号、年金番号、保険者番号、顔認証データ、指紋認証データなど）
2.　当社が取得する個人情報には、例えば、以下のようなものがあります。
氏名、性別、生年月日、住所、郵便番号、電話番号、ファックス番号、電子メールアドレス
勤務先の名称、所属部署名、役職名、住所、郵便番号、電話番号、ファックス番号、電子メールアドレス
クレジットカード情報、銀行口座情報
パスポート番号、免許証番号、年金番号、保険者番号その他の身分証明書番号
商品の購入履歴、サービスの利用履歴、ウェブサイトへの通信履歴（閲覧履歴、検索履歴等）
病歴、健康診断や検査の結果、質問等への回答結果、診療や薬剤の処方に関する記録、その他の健康情報（以下「健康情報」といいます。）
撮影・録音された個人の画像や音声
会員資格に関する情報（会員の有無、入退会の履歴等）
3.　「個人情報」には、「統計情報」は含まれません。
「統計情報」とは、複数の個人の情報から、商品・サービス・会員等のグループやカテゴリーごとに共通する要素を抽出して数値化したデータで、特定の個人に関する情報を含まないものです。「統計情報」は、当社がトレンドと会員等のニーズを理解するために用いられ、このことにより、当社は新しい商品やサービスについてより良い検討を行うことができるとともに、現存する商品及びサービスを会員等の要望にお応えするよう改善できることになります。

B.　個人情報の取得
1.　当社は、適法かつ適正な手段により、会員等の個人情報を取得します。
当社が取得する個人情報には、会員等のご本人の個人情報に加えて、会員等を通じて間接的に取得したご家族等の個人情報も含まれます。

2.　当社は、主に以下の場合に個人情報を取得します。
当社の会員に登録する場合、又は当社の会員資格を更新する場合
当社の会員サービスに登録し、または参加される場合
当社の商品やサービスを注文し、または購入する場合
アンケート等に回答する場合
当社の倉庫店等に来店する場合
当社の会員サービス、イベントまたは販売促進キャンペーンに関するお問合わせや連絡をいただく場合
薬局、倉庫店等で質問表等に記入する場合
当社以外の第三者から委託、事業承継または共同利用により個人情報が提供される場合
3.　要配慮個人情報の取得
（1）「要配慮個人情報」とは、ご本人の人種、信条、社会的身分、病歴、犯罪の経歴、犯罪により害を被った事実その他本人に対する不当な差別、偏見その他の不利益が生じないようにその取扱いに特に配慮を要する記述等が含まれる個人情報をいいます。

（2）当社は、以下の場合を除いて、あらかじめご本人の同意を得ないで要配慮個人情報を取得しません。

法令に基づく場合
人の生命、身体又は財産の保護のために必要がある場合であって、ご本人の同意を得ることが困難であるとき。
公衆衛生の向上又は児童の健全な育成の推進のために特に必要がある場合であって、ご本人の同意を得ることが困難であるとき。
国の機関若しくは地方公共団体又はその委託を受けた者が法令の定める事務を遂行することに対して協力する必要がある場合であって、ご本人の同意を得ることにより当該事務の遂行に支障を及ぼすおそれがあるとき。
要配慮個人情報が、ご本人、国の機関、地方公共団体等により公開されている場合
ご本人を防犯カメラ等により撮影する場合
要配慮個人情報を委託、事業の承継または共同利用により取得する場合
（3）当社は、会員等から会員等のご家族の要配慮個人情報を取得する場合には、原則として、ご家族の同意を得て取得します。
なお、会員等のご家族が年少、ご高齢、ご病気等のために同意するかどうかについて判断できる能力がない場合又は会員等のご家族が会員等に対して当社に要配慮個人情報を提供することへの承諾を与えている場合には、会員等から同意を得て要配慮個人情報を取得する場合があります。

C.　個人情報の利用目的
1.　当社は、会員等の個人情報（健康情報については、２のとおり）を、以下の目的の範囲内で利用します。
会員等からの商品やサービスの注文への対応・確認・記録
会員等が購入した商品の配送
会員等向けのイベント、販売促進キャンペーン、割引等のプロモーションに関する情報（以下「プロモーション情報」といいます。）を提供するため
第三者が提供する会員等向けの特別の商品やサービス（以下「会員サービス」といいます。）を紹介、提供するため
会員等のご意見やご要望を把握するためのアンケート等の実施
クレジットカード等による代金決済、年会費等の自動引き落としのため
商品のリコールまたは安全上の問題について会員等に警告するため
商品の不具合に関するお問い合わせ、修理、交換に対応するため
会員等からのお問い合わせ等に対応するため
商品やサービスの企画・開発・提案のための個人情報の分析
会員等が個別に同意した目的に利用するため
その他、当社がより良いサービスを提供することができるように会員等のニーズを理解するため
当社が会員等と締結している契約に関する義務の履行や契約関係の維持・管理等のため
当社、当社の会員、当社の従業員もしくは第三者の生命・身体の安全、財産その他の権利を保護するため
2.　当社は、会員等の個人情報のうち健康情報については、以下の目的の範囲内で利用します。
調剤サービス及び薬剤情報のご提供
健康維持及び増進等に有益と思われる情報のご提供
保険請求事務
投薬に際してのご家族等に対する情報提供を行うため
調剤サービス及び保険請求事務処理に際し、処方箋発行元の医療機関又は医師へ照会するため
病院、診療所、薬局、訪問看護ステーション、介護サービス事業者などとの必要な連携を図るため
保険請求事務に係る審査支払機関に対する調剤報酬明細書の提出のため
保険請求事務に係る審査支払機関又は保険者からの照会に対する回答のため
メガネ、コンタクトレンズ、補聴器等の作成、提供、メンテナンス等のため
会員等からのお問い合わせ等に対応するため
情報分析等の研究のため（個人を特定できないように匿名加工した情報として利用します）
当社が会員等と締結している契約に関する義務の履行や契約関係の維持・管理等のため

Ｄ.　個人情報の第三者提供
1.　本人の同意に基づく第三者提供
当社は、以下の場合、ご本人の同意に基づいて、「Ｃ．個人情報の利用目的」の範囲内で、会員等の個人情報を第三者に提供することがあります。
（委託または共同利用に伴って個人情報を委託先や共同利用者に提供する場合については、「Ｅ．個人情報の取扱いの委託」及び「Ｆ．個人情報の共同利用」をご覧ください。）

会員等が購入した商品のベンダー（販売会社）・サプライヤー（製造会社）等に、送付先や請求先の住所、電子メールアドレス、電話番号及び商品オーダー等の、会員等が注文する商品又はサービスを提供するために必要な個人情報を提供する場合
会員等からのお問い合わせ等に対応するため、ベンダー・サプライヤー等に連絡・照会等をする場合
ご本人に代わって個人情報を第三者に提供する場合
（例）
・会員等がクレジットカードの発行を申し込むために、当社を通じてクレジットカード会社に申込みに必要な個人情報を提供する場合
・クレジットカードで代金を決済するのに伴い、当社を通じてカード番号や売上・取引に関する情報等をクレジットカード会社や個人信用情報機関に提供する場合
・年会費等の自動引き落としのためにシステム会社に必要な個人情報を提供する場合

「Ｃ．個人情報の利用目的」・２に記載した利用目的のために、健康情報を他の病院、診療所、薬局、訪問看護ステーション、介護サービス事業者、審査支払機関または保険者等に提供する場合
その他、「Ｃ．個人情報の利用目的」記載の利用目的を達成するために第三者に個人情報を提供する必要がある場合
外国にある第三者に、1)から5)の事由により個人情報を提供する場合

2.　法令に基づく場合などの第三者提供
当社は、以下の場合に、ご本人の同意を得ないで、個人情報を第三者に提供することがあります。

法令に基づく場合
人の生命、身体又は財産の保護のために必要がある場合であって、本人の同意を得ることが困難であるとき（例えば、会員等が当社で購入した商品に製品事故が生じて、製造メーカーよりリコールの要請があった場合に、その商品を購入した会員等の連絡先等の情報を製造メーカーに提供する場合など。）
公衆衛生の向上又は児童の健全な育成の推進のために特に必要がある場合であって、本人の同意を得ることが困難であるとき
国の機関若しくは地方公共団体又はその委託を受けた者が法令の定める事務を遂行することに対して協力する必要がある場合であって、本人の同意を得ることにより当該事務の遂行に支障を及ぼすおそれがあるとき
Ｅ.　個人情報の取扱いの委託
1.　当社は、会員等の個人情報の取扱いを外部業者に委託することに伴い、個人情報を委託先に提供することがあります。その場合、適切な委託先を選定し、委託先に対して、委託された個人情報の安全管理が図られるように指示、監督します。
2.　当社が個人情報の取扱いを外部業者に委託する場合には、例えば、以下のような場合があります。

また、事業の承継に先立つ交渉過程において、適切な情報管理の措置を講じた上で個人情報を事業の承継の相手方に提供する場合があります。

Ｈ.　安全管理措置
当社は、個人情報が当社の管理下にある間の、喪失、誤使用又は改ざんから個人情報を保護するよう、安全管理措置に努めています。当社が収集する個人情報は、電磁的に保管されており、他の会員情報と組み合わされることがあります。当社は、無権限のアクセスから個人情報を保護するよう、技術上、契約上、管理上及び物理上の措置を取るよう努めています。

Ｉ.　オンラインに特有の取扱い
1.　クッキーとは何か、クッキーを認めることが必要か
クッキーは、会員等のコンピュータに保存される小さなファイルです。costco.co.jpにおいて、当社は、会員等の履歴を最適化するために、クッキーを使用しています。costco.co.jpは、いかなる個人情報もクッキーにより保存することはありません。 costco.co.jp をブラウズするクッキーの選択は自由です。

2.　他のウェブサイトへのオンライン上のリンク
会員サービスおよび当社ウェブサイトの選択された他の部分を通じて、会員等は、当社の会員等に商品およびサービスを提供することに同意した第三者の他のウェブサイトにリンクすることができます。会員等が当該リンクされたページで提供された個人情報は、当該第三者に直接提供され、当該第三者のプライバシーポリシーが適用されます。上記に記載された場合を除き、当社は、当社がリンクするウェブサイトのコンテンツ、プライバシーおよびセキュリティの運用、ポリシーにつき責任を負いません。当社のサイトから第三者のサイトや他のサイトへのリンクは、会員等の利便性のために提供されます。会員等は、当該第三者に個人情報を提供する前に、当該第三者の個人情報保護及びセキュリティに関する取扱い及び方針に精通されることをお勧めします。

3.　当社がオンライン上で収集する情報の保有
会員等のオンラインでの活動の結果として当社が自動的に収集する情報は、一般には個人を特定できる形で保有されることはなく、もし当該情報が個人を特定できるものである場合には、当社は当社のウェブサイトに関連する研究および分析を行うために必要な期間のみ、これを保有します。ただし、特定の取引やイベントに関連する不正や問題を調査するために必要な情報については、この限りではありません。

4.　Googleアナリティクスの利用について
当サイトでは、利用状況を把握してより良いホームページをご提供するため、Googleアナリティクスを利用しています。Googleアナリティクスはクッキーを使用して利用状況を収集しますが、これには個人を特定する情報は含まれておりません。Googleアナリティクスに関してはGoogleアナリティクスのサイト（http://www.google.co.jp/analytics/）をご確認ください。また、よりよいサービスをご提供するため、GoogleアナリティクスでGoogleのインタレストベース広告のデータや第三者のユーザーデータ（年齢、性別、興味や関心など）を利用しております。

Ｊ.　個人情報の開示、訂正、利用停止等
当社は、会員等による自己の個人情報の利用目的の通知、開示、内容の訂正、追加、削除、利用の停止、消去、第三者への提供の停止等の求め（以下「開示等の求め」といいます。）に対して、以下に掲げる手続に従って、対応します。

1.　開示等の求めの受付先
開示等の求めは、当社の国内各倉庫店のメンバーシップ・カウンターにおいて、ご本人または代理人（代理権を確認できた場合に限ります。）からの求めに対してのみ受け付けます。

2.　開示等の求めに際して提出する書面の様式、その他の開示等の求めの受付方法
当社は、書面の形式によってのみ個人情報の開示等の求めに応じます。
個人情報を不正行為から保護するために、当社は、郵送、ファクシミリ、電話又は電子メールによる請求には応じることはできません。

3.　開示等の求めをする者が本人又はその代理人であることの確認方法
いずれの開示等の求めについても、運転免許証、パスポート又はその他の顔写真付き身分証明書が必要で、会員の場合にはさらにコストコ会員証が必要です。

Ｋ.　個人情報保護方針の変更
当社は、いつでも、この個人情報保護方針を変更する権利を留保しています。当社は、提供された個人情報がどのように利用されるかを確実にご理解いただくために、当社の個人情報保護方針を確認することをお勧めします。

1.　“Personal information” in this policy on the protection of personal information refers to “personal information” that is specified in the Act on the Protection of Personal Information of Japan, and the following information is included in “personal information.”

3.　“Personal information” does not include “statistical information.”
“Statistical information” is data obtained by extracting common elements for each group or category of products, services, members or the like from information of multiple individuals and quantifying the extracted elements, and does not include information on particular individuals. “Statistical information” is used by our company to understand the trends and the needs of our members or the like, and with such understanding, we can conduct better studies of new products and services, and improve the existing products and services in order to respond to the requests of our members or the like.

B.　Acquisition of personal information
1.　Our company acquires personal information of our members or the like by legitimate and appropriate means.
Personal information that our company acquires includes not only personal information of our members or the like themselves, but also personal information of their families, etc. that our company indirectly acquires through our members or the like.

2.　Our company acquires personal information mainly in the following cases.
When a person is registered as a member of our company, or when a person’s membership of our company is renewed
When a person is registered for our company’s member services, or participates in the services
When a person places an order for, or purchases, our company’s product or service
When a person replies to a questionnaire or the like
When a person visits our company’s warehouse or the like
When a person makes an inquiry on our company’s member services, events or promotional campaigns
When a person fills in a questionnaire or the like at a pharmacy, warehouse, etc.
When a third party other than our company provides personal information on a commissioned basis, or through business succession or joint use
3.　Acquisition of special-care required personal information
（1）“Special-care required personal information” refers to personal information comprising the principal’s race, creed, social status, medical history, criminal record, or the fact of having suffered damage by a crime, or other descriptions, etc. as those of which the handling requires special care so as not to cause unfair discrimination, prejudice or any other disadvantages to the principal.

（2）Our company does not acquire special-care required personal information without obtaining the principal’s consent in advance, except in the following cases.

Cases based on laws and regulations
Cases in which there is a need to protect a human life, body or fortune, and when it is difficult to obtain the principal’s consent
Cases in which there is a special need to enhance public hygiene or promote fostering healthy children, and when it is difficult to obtain the principal’s consent
Cases in which there is a need to cooperate in regard to a central government organization or a local government, or a person entrusted by them performing affairs prescribed by laws and regulations, and when there is a possibility that obtaining a principal’s consent would interfere with the performance of the said affairs
Cases in which the special-care required personal information is being open to the public by a principal, a central government organization, local government or the like
Cases in which the principal is photographed with a security camera or the like
Cases in which the special-care required personal information is acquired on a commissioned basis, or through business succession or joint use
（3）If our company acquires special-care required personal information of a family of our member or the like from the member or the like, our company obtains the family’s consent in advance in principle.
If a family of the member or the like does not have the ability to determine whether or not to consent due to his/her juvenility, advanced age, illness, etc., or the family of the member or the like has given the member or the like an approval to provision of such special-care required personal information, we may acquire such special-care required personal information with the consent of the member or the like.

C.　Purpose of use of personal information
1.　Our company uses personal information (for health information, Section 2 shall apply) of our members or the like within the scope of the following purposes.
Response to, or confirmation or recording of an order for our product or service from our member or the like
Delivery of a product purchased by our member or the like
Provision of information on promotions such as an event for our members or the like, a promotional campaign, or a discount, etc. (hereafter called “promotion information”)
Introduction to or provision of a special product or service for our members or the like (hereafter called “member service”) that a third party provides
Conduct of a questionnaire or the like intended to grasp opinions and requests of our members or the like
Payment with a credit card or the like, and automatic withdrawal of an annual membership fee, etc.

In the following cases, our company may provide personal information of our members or the like to a third party within the scope of “C. Purpose of use of personal information” based on consent of the principal.

The costco.co.jp website does not store any personal information using cookies. Our members or the like have the option not to choose cookies for browsing “costco.co.jp.”

2.　On-line links to other websites
Any of our members or the like can link to a website of a third party that agrees to provide their products and services to our members or the like through the other selected parts of our member services and our company’s website. Personal information that our member or the like provides on such linked page is directly provided to such third party, and a privacy policy of the third party will be applied. Except for the cases indicated above, our company will not assume responsibility for the contents, privacy and security operation and policy of the websites that our company links. The links from our company’s website to third-party sites and other sites are provided for convenience of our members or the like. We recommend that our members or the like become acquainted with any such third-party’ handling of and policy for personal information protection and security before providing their personal information to the third party.

3.　Retention of information that our company collects on-line
The information that our company automatically collects as a result of on-line activities of members or the like is generally not retained in a manner that the individual can be identified. If such information enables the identification of an individual, our company will retain the information for only a period required to conduct a study or analysis concerning our company’s website. However, this does not apply to information required to investigate fraud or problems related to specific transactions and events.
